A History of Fluoride: How It Became Integrated into Everyday Life

Today, fluoride is a familiar part of preventive dental care. It’s found in many toothpastes and, in some communities, added to drinking water to help prevent tooth decay. But how did fluoride become such an important part of modern dentistry? Its history began with an unexpected observation about teeth.

An Unexpected Discovery

In the early 20th century, dentist Frederick McKay moved to Colorado Springs, Colorado to open a dental practice and soon noticed that many people had unusual brownish discoloration on their teeth. McKay became interested in this condition, which eventually became known as dental fluorosis.

Researchers later discovered that the discoloration was associated with naturally occurring fluoride in drinking water. An important discovery followed: people with mild fluorosis often appeared to have fewer cavities.

In the 1930s, researchers, including H. Trendley Dean of the U.S. Public Health Service, investigated the relationship between fluoride levels in drinking water, fluorosis, and tooth decay. Their research helped establish that there was a level of fluoride that could provide protection against cavities while minimizing the risk of noticeable fluorosis.

Fluoride Comes to Drinking Water

This research eventually led to a major public health milestone. In 1945, Grand Rapids, Michigan, became the first city in the world to intentionally fluoridate its community water supply.

Researchers monitored dental health in Grand Rapids and compared it with nearby communities without fluoridated water. The results were astounding: after 11 years, the rate of cavities in children who were born after the water supply was fluoridated dropped by 60 percent.

Water fluoridation subsequently expanded throughout the United States. In 1962, the U.S. Public Health Service established recommended fluoride concentrations for community water systems, helping standardize the practice.

Today, community water fluoridation remains one of the most widely used public health measures for reducing tooth decay, although recommendations and practices can vary by location.

How Fluoride Entered Toothpaste

Fluoride’s benefits weren’t limited to drinking water. Scientists also began investigating whether putting fluoride directly on teeth could help prevent cavities.

Early fluoride toothpastes were developed in the mid-20th century. A major milestone occurred in 1955, when the American Dental Association recognized Crest as the first fluoride toothpaste demonstrated to be effective in reducing cavities.

Over the following decades, fluoride became a standard ingredient in most conventional toothpastes.

Fluoride’s Role Today

Fluoride helps strengthen tooth enamel and makes teeth more resistant to acid attacks caused by bacteria and dietary sugars. It can also help reverse or remineralize early stages of tooth decay before a cavity becomes established. No wonder it’s become a common treatment in preventive dental services.

For most people, brushing twice daily with fluoride toothpaste and drinking fluoridated water when available can provide valuable protection against cavities.

What began as an investigation more than a century ago into strangely discolored teeth ultimately transformed preventive dentistry. From naturally occurring fluoride discoveries to community water fluoridation and fluoride toothpaste, research has helped make tooth decay more preventable.
